By simplifying the preparation of insurance

delivery of good medical care. Potential problems with billing may
be avoided by thoroughly completing your patient history sheet.
Please inform our receptionist of any changes since your last visit,
and bring your insurance cards and/or claim forms with you at the
time of your appointment.
Please remember that insurance is considered a
method of reimbursing the patient for fees paid to the doctor, and
is not a substitute for payment. Some companies pay fixed
allowances for certain procedures, and others pay a percentage of
the charge. It is your responsibility to pay any deductible amount,
coinsurance, or any other balance not paid by your insurance
company.
